You will be part of a leading organisation that values innovation and excellence in community health services. Our team is committed to improving the quality of life for our patients and their families, and we are looking for a motivated and caring professional to join us.
The successful candidate will have relevant experience in healthcare and excellent clinical, managerial, and communication skills. They will be able to work autonomously, supporting the team lead to manage the caseload and staff development.
Main Responsibilities
The post-holder will be responsible for managing a defined community caseload, providing nursing, physiotherapy, or occupational therapy interventions as part of the Integrated Care Team. They will coordinate and delegate work, ensuring referrals are dealt with according to clinical need, and that holistic assessments and care plans are recorded in line with Trust and professional standards.
The post-holder will work with a high degree of autonomy and professional accountability, aiming to prevent unnecessary hospital admissions and support timely discharge into the community. They will also be responsible for re-enablement, rehabilitation, health protection, and promotion programs that improve and reduce inequalities.

Person Specification
The successful candidate will be NMC registered and have relevant experience in healthcare. They will have excellent clinical, managerial, and communication skills, and be able to work autonomously, supporting the team lead to manage the caseload and staff development.
