Position Overview

Shakespeare Road PMS is seeking a dedicated and skilled Senior Nurse Practitioner to join our collaborative team of healthcare professionals. This part-time role encompasses 15 hours per week, distributed over three days, providing an opportunity to contribute to patient care in a supportive environment.

Key Responsibilities

  • Assess and manage patients with various acute medical issues.
  • Develop and assess tailored treatment plans for patients with chronic conditions.
  • Identify and support patients at risk of long-term health issues, aiming to prevent negative health outcomes.
  • Order and interpret laboratory tests as necessary.
  • Maintain precise clinical documentation.
  • Prioritize health concerns and take appropriate action.
  • Recognize and refer patients with mental health challenges.
  • Act as an extended and supplementary prescriber.
  • Contribute to practice performance targets, adhering to local and regional guidelines.
  • Collaborate with external agencies to ensure comprehensive patient support.
  • Assist the clinical team with safeguarding responsibilities, following established policies.

Shakespeare Road PMS operates as a dual-site GP Training Practice, serving a diverse patient population. Our team comprises experienced professionals, including GP Partners, Nurse Practitioners, Practice Nurses, and other healthcare specialists, all committed to delivering high-quality care.

Established in 1987, we pride ourselves on our strong performance in quality outcomes and our participation in enhanced services. Our practice utilizes advanced clinical systems and maintains positive relationships with neighboring practices, forming part of a successful Primary Care Network.

Job Duties

  • Design and implement health promotion initiatives.
  • Provide expert advice and care for patients with acute and chronic medical conditions.
  • Conduct chronic disease management clinics, ensuring appropriate referrals to specialist services.
  • Maintain accurate records and ensure effective use of clinical coding.
  • Support the team in managing clinical emergencies.
  • Implement vaccination programs for various age groups.
  • Deliver health promotion interventions as appropriate.

Qualifications

Required

  • Registered Nurse with NMC.
  • V300 Prescribing qualification.
  • Must possess or be able to obtain a digital badge prior to commencement.

Experience

Required

  • Proven experience as an Advanced Nurse Practitioner.
  • Background in General Practice is essential.
  • Experience in assessing and managing minor injuries and illnesses.
  • Demonstrated ability to work both independently and as part of a team.
