Position Overview

Islington GP Federation is in search of a Clinical Lead specializing in Long-Term Conditions (LTC) to spearhead a transformative care model and deliver training and clinical support to general practice teams within the area as part of the LTC Locally Commissioned Service (LCS).

This role is structured for a commitment of 8 hours per week, with flexibility for job sharing to incorporate diverse insights into the initiative. The successful candidate will work closely with the Business Change Facilitator and the LTC LCS programme team to ensure effective support delivery.

Key Responsibilities

The primary objective of this program is to integrate a comprehensive model of personalized care within primary care settings, particularly in general practice.

The Clinical Lead will:

  • Champion the personalized care model and provide essential training and clinical guidance to GP practices for the new LTC LCS.
  • Engage proactively with stakeholders, including Primary Care Networks and general practices, to enhance business processes and support improvements across North Central London.
  • Facilitate the understanding and implementation of proactive management strategies for practice lists, promoting personalized care approaches.
  • Advocate for the Long-Term Condition Service to ensure robust support from all practices and Primary Care Networks.
  • Offer guidance to align business change activities with clinical practice enhancements.

Specific Responsibilities

The Clinical Lead will:

  • Engage with practices to enhance their understanding of the clinical requirements of the LTC LCS model and the necessary changes for effective delivery.
  • Participate in practice, PCN, and borough meetings to discuss the LCS, providing information and training tailored to local needs.
  • Collaborate with change facilitators to assist practices and primary care networks in embedding a cycle of reviewing and acting on LCS outcome data.
  • Document and share practice insights, feedback, and examples of effective practices, addressing challenges as needed.

Qualifications and Skills

The ideal candidate will possess:

  • Clinical experience in managing long-term conditions within a General Practice environment.
  • Expertise in the LTC LCS, including both the clinical model and necessary change management practices.
  • Experience in enhancing non-clinical processes that support long-term condition management.
  • Strong analytical skills and the ability to interpret data to drive improvement actions.
  • Excellent communication skills, with the ability to build credibility and engage effectively with General Practice teams.
