Opportunity for a Clinical Coordinator - Long Term Conditions

We are seeking a skilled Clinical Coordinator to join our team at UCLPartners, working on a 6-month fixed-term secondment opportunity with the possibility of extension.

The successful candidate will work with early adopter sites and a small team to develop systems and processes to support remote review of High Risk and Complex patients for the Primary Care Network (PCN), building on their experience of working with individuals with Long Term Conditions.

Key Responsibilities:

  • Develop and implement systems and processes to support remote review of High Risk and Complex patients
  • Work with early adopter sites and a small team to design and test an improved model of care for Long Term Conditions
  • Collaborate with the Integrated Care Board (ICB) to run an Long Term Conditions programme
  • Support coordination of care across acute, community, and mental health services for complex patients

Requirements:

  • Band 8a, Allied Health Professional (AHP), nurse, clinical pharmacist, physician associate, or manager with clinical/AHP background
  • Experience working with individuals with Long Term Conditions
  • Strong communication and collaboration skills

Working Arrangements:

  • Full-time (1 WTE) or part-time with a minimum of 0.5 WTE
  • Remuneration at Band 8a or equivalent
