We are seeking a highly skilled and dedicated individual to join our team as a Long Term Conditions Specialist. This is an exciting opportunity to work with our experienced LTC consultant and clinical coordinator, supporting the gathering of information and case management.
Key Responsibilities- Support the LTC consultant and clinical coordinator in gathering information and case management.
- Organize regular case discussions between the LTC team and PCN representatives to ensure effective collaboration and communication.
- Establish and maintain regular contact with teams across Trusts and a range of GP practices to facilitate effective working relationships.
- Monitor and follow-up on outcomes to ensure the highest level of patient care and satisfaction.
- Understand and promote patient and public involvement in our work, ensuring that their voices are heard and valued.
- Commit to equity, diversity, and inclusion principles, applying them to this role to ensure a fair and inclusive working environment.
- Strong understanding of long-term conditions and their impact on patients and healthcare services.
- Excellent communication and interpersonal skills to work effectively with diverse stakeholders.
- Ability to work independently and as part of a team, prioritizing tasks and managing multiple responsibilities.
- Strong organizational and time management skills to meet deadlines and deliver high-quality results.
