About the Role

We are seeking a highly skilled and motivated Specialist Clinical Pharmacist to join our team at Guy's and St Thomas' NHS Foundation Trust. The successful candidate will have a strong background in hospital pharmacy practice and a demonstrable commitment to pharmaceutical care.

Key Responsibilities

  • Provide safe, clinically effective, and cost-efficient use of drugs in patients with long-term conditions.
  • Develop and implement clinical pharmacy services in line with local and national objectives.
  • Work collaboratively with pharmacy and medical staff, the multidisciplinary team, and management to deliver high-quality patient care.
  • Act as a role model for junior pharmacists, pre-registration pharmacists, pharmacy technicians, and students in the rotation.
  • Implement agreed clinical governance initiatives and promote best practice.
  • Manage patients' pharmaceutical needs appropriately and make referrals as necessary.
  • Ensure the quality of patient care and demonstrate professional accountability.
  • Document the workload and quality of pharmacy services in line with departmental and corporate objectives.
  • Undertake clinical audit and service evaluation to improve clinical patient outcomes.

Requirements

  • Vocational masters degree (MPharm) and successful completion of short courses to the equivalent of a Postgraduate Diploma in General Pharmacy Practice.
  • Professional Registration with GPhC and experience in hospital pharmacy practice with assessed competencies in dispensary services, medicines information, and technical services.
  • Demonstrable ability to ensure safe and effective use of medicines for patients in medical and surgical specialties.
  • Demonstrable ability to respond to medicines incidents and manage patients independently in anticoagulation clinics.

Desirable Qualifications and Experience

  • Registered independent prescriber and structured training rotation.
  • Demonstrable experience of managing patients independently in anticoagulation clinics and clinically influencing at senior level ward rounds.
