About the Role

We are seeking a highly skilled and experienced Consultant Advanced Clinical Practitioner to join our East Kent HCP Frailty/Home Treatment Service. As a key member of our team, you will play a crucial role in providing high-quality clinical care to older people in a variety of healthcare settings.

Key Responsibilities
  • Apply advanced clinical expertise, levels of judgment, discretion, and decision-making to the provision of clinical care to older people.
  • Exercise a high degree of autonomy through the application of advanced clinical skills in relation to the assessment, diagnosis, treatment, and management of older people.
  • Lead and monitor improvements to quality of care through clinical audit, evidence-based practice, clinical supervision, teaching, and supporting professional colleagues.
  • Implement relevant clinical guidelines into practice and contribute to the further development of the evidence base through research and audit.
  • Develop non-medical led models of care across emergency departments, inpatient wards, intermediate care, care homes, and community teams in collaboration with multi-professional colleagues.
  • Improve patient flow through timely assessment, referral, responsive intervention, anticipatory care planning, and optimal discharge coordination across seven days.

Requirements
  • First-level registration with NMC, HPCP, or GPHC.
  • Minimum of 5 years post-registration experience, with experience at band 8a or above.
  • Advanced Clinical Practice MSc accredited by the Centre for Advancing Practice or successful recognition with the Centre of Advancing Practice via the ePortfolio (Supported) Route.
  • Maintained a portfolio of ongoing evidence or work-based learning.
  • Teaching qualification.
  • Research qualification.
  • Leadership.
  • Non-medical prescribing.
Desirable Qualifications and Experience
  • Psychology or counseling experience and training/education.
  • Extensive experience of presenting reports/education/information to public forums.
  • IR(ME)R training or equivalent.
