Job Overview

We are seeking a dedicated Band 3 Senior Healthcare Assistant to become part of our Urgent Care Service.

Our Urgent Care Services deliver immediate health, therapy, and care assistance to help individuals stay within their community or home settings. This includes providing care for individuals experiencing acute health issues that might otherwise necessitate hospital admission, as well as facilitating safe transitions from hospital to home. Our service operates 24/7 throughout the year.

At Kent Community Health NHS Foundation Trust, we collaborate with East Kent Hospitals University Trust to innovate a virtual ward for patients with acute respiratory conditions, such as chest infections, or those with chronic conditions like COPD, allowing them to receive care in their homes.

Patients who opt for early discharge from the hospital will be monitored at home through advanced technology, including applications, wearable devices, or other remote medical tools, and will receive support from a consultant, specialist GP, or advanced practitioner. The specific technologies to be utilized are currently being determined as we work alongside patients and colleagues to design the wards in the upcoming months.

Key Responsibilities

As a Band 3 Senior Healthcare Assistant, your role will include:

  • Delivering components of prescribed care packages under the oversight of a Nurse, Therapist, or Healthcare Practitioner.
  • Contributing to the ongoing assessment and evaluation of delegated caseloads, reporting all clinical and behavioral outcomes to the supervising nurse or therapist to facilitate re-assessments.
  • Performing and documenting delegated clinical tasks that have been previously authorized, including baseline vital signs, routine urinalysis, glucose monitoring, wound care, falls prevention, and equipment provision, while promoting independence through daily living activities.
  • Collaborating effectively as part of a multi-disciplinary team to ensure high-quality care for patients.
  • Planning and executing straightforward tasks in accordance with established operating procedures and protocols, undertaking roles and responsibilities as assigned by a nurse or therapist.
  • Conducting clinical interventions using various methods, including leading group and individual therapy sessions as part of prescribed care packages after achieving the necessary competencies as delegated by a nurse or therapist.

Please note that due to the nature of visiting patients in their homes, access to a vehicle is required for this position.

Person Specification

Qualifications

Essential

  • Relevant level 3 qualification (NVQ/QCF Diploma or certificate of care/Advanced apprenticeship or equivalent or working towards) and Functional Skills at Level 2 (Literacy and numeracy/GCSE grade A-C or equivalent in English and Maths).

Experience

Essential

  • Minimum of 2 years' experience as a Healthcare Assistant/Worker in a health or social care environment.
  • Experience working collaboratively within a team.
  • Proficiency in Microsoft Office and various IT applications (E-Pay/E-roster/AT-P/CIS).

Desirable

  • Experience in a community nursing setting.

Knowledge

Essential

  • Awareness of developments within the NHS.
  • Knowledge of current clinical practices.
  • Understanding of the role and responsibilities of unregistered workers.

Skills & Abilities
