Clinical Practitioner Specialist

We are seeking a highly skilled Clinical Practitioner Specialist to join our East Kent HCP Frailty/Home Treatment Service at Kent Community Health NHS Foundation Trust. As a key member of our team, you will provide advanced clinical expertise, exercising high levels of judgement, discretion, and decision-making to deliver exceptional care to older people in various healthcare settings.
Main Responsibilities- Lead and Monitor Quality of Care
- Develop and implement quality improvement initiatives to enhance patient care and outcomes.
- Conduct clinical audits to identify areas for improvement and develop strategies to address them.
- Provide clinical supervision and guidance to colleagues to ensure high-quality patient care.
- Implement Clinical Guidelines
- Develop and implement evidence-based clinical guidelines to improve patient care and outcomes.
- Contribute to the development of the evidence base through research and audit.
- Develop Non-Medical Led Models of Care
- Design and implement non-medical led models of care to improve patient outcomes and experience.
- Collaborate with multi-professional colleagues to develop and implement these models.
- Improve Patient Flow
- Develop and implement strategies to improve patient flow through timely assessment, referral, and discharge coordination.
- Work with colleagues to identify and address barriers to patient flow.

Person Specification- Qualifications
- First-level registration with NMC, HPCP, or GPHC.
- Minimum of 5 years post-registration experience, experience at band 8a or above.
- Advanced Clinical Practice MSc accredited by the Centre for Advancing Practice or successful recognition with the Centre of Advancing Practice via the ePortfolio (Supported) Route.
- Maintained a portfolio of ongoing evidence or work-based learning.
- Teaching qualification.

- Non-medical Prescribing.
- Experience
- Extensive clinical experience in the care and treatment of people with frailty (including long-term conditions and dementia care experience).
- Advanced skills in history taking and physical assessment of frail people.
- Independent prescribing experience.
- Experience of advanced specialist interventions in the assessment and management of older patients.
- Experience of teaching and education provision.
- Extensive experience in report writing.
- Experience of planning, implementation, and delivery of educational programmes relating to long-term conditions.
- Experience of working across organisational boundaries with partners (commissioning, social care, and education providers).
- Advanced IT skills.
- Experience with evidence of audit, research, and innovation applied in practice.
