About the Role

We are seeking a highly skilled Clinical Practitioner Specialist to join our East Kent HCP Frailty/Home Treatment Service at Kent Community Health NHS Foundation Trust. As a key member of our team, you will provide advanced clinical expertise, exercising high levels of judgement, discretion, and decision-making to deliver exceptional care to older people in various healthcare settings.

Main Responsibilities
  • Lead and Monitor Quality of Care
    • Develop and implement quality improvement initiatives to enhance patient care and outcomes.
    • Conduct clinical audits to identify areas for improvement and develop strategies to address them.
    • Provide clinical supervision and guidance to colleagues to ensure high-quality patient care.
  • Implement Clinical Guidelines
    • Develop and implement evidence-based clinical guidelines to improve patient care and outcomes.
    • Contribute to the development of the evidence base through research and audit.
  • Develop Non-Medical Led Models of Care
    • Design and implement non-medical led models of care to improve patient outcomes and experience.
    • Collaborate with multi-professional colleagues to develop and implement these models.
  • Improve Patient Flow
    • Develop and implement strategies to improve patient flow through timely assessment, referral, and discharge coordination.
    • Work with colleagues to identify and address barriers to patient flow.

Person Specification
  • Qualifications
    • First-level registration with NMC, HPCP, or GPHC.
    • Minimum of 5 years post-registration experience, experience at band 8a or above.
    • Advanced Clinical Practice MSc accredited by the Centre for Advancing Practice or successful recognition with the Centre of Advancing Practice via the ePortfolio (Supported) Route.
    • Maintained a portfolio of ongoing evidence or work-based learning.
    • Teaching qualification.
    • Research qualification.
    • Leadership.
    • Non-medical Prescribing.
  • Experience
    • Extensive clinical experience in the care and treatment of people with frailty (including long-term conditions and dementia care experience).
    • Advanced skills in history taking and physical assessment of frail people.
    • Independent prescribing experience.
    • Experience of advanced specialist interventions in the assessment and management of older patients.
    • Experience of teaching and education provision.
    • Extensive experience in report writing.
    • Experience of planning, implementation, and delivery of educational programmes relating to long-term conditions.
    • Experience of working across organisational boundaries with partners (commissioning, social care, and education providers).
    • Advanced IT skills.
    • Experience with evidence of audit, research, and innovation applied in practice.
    • Experience of strategy development and implementation.
