About the Role
We are seeking an Advanced Clinical Practitioner to lead our East Kent HCP Frailty/Home Treatment Service. The successful candidate will apply advanced clinical expertise, levels of judgement, discretion and decision making to the provision of clinical care to older people within a variety of healthcare settings.
Main Responsibilities
The Advanced Clinical Practitioner will lead and monitor improvements to quality of care through clinical audit, evidence-based practice, clinical supervision, teaching and supporting professional colleagues. They will also lead the implementation of relevant clinical guidelines into practice and contribute to the further development of the evidence base through research and audit.

Person Specification
The successful candidate will have professional registration with NMC, HCPC or GPhC, evidence of post-registration study and teaching qualification. They will also have knowledge of research process and governance, audit process, management of patients with long-term conditions and complex needs, local and national health policy and knowledge of public health issues.
Skills and Abilities
The Advanced Clinical Practitioner will be proficient in Microsoft Office and clinical data systems, have excellent communication and interpersonal skills and be able to undertake risk assessment. They will also have autonomous practitioner skills, clinical examination skills and advanced clinical practice skills.

Ability to Manage
The Advanced Clinical Practitioner will be able to manage a caseload, cope with competitive demands in stressful situations and manage change effectively.
