About the Role

We are seeking an experienced Public Health Advanced Practitioner to join our team at Sussex Community NHS Foundation Trust. As a key member of our East Kent HCP Frailty/Home Treatment Service, you will play a crucial role in delivering high-quality patient care and improving health outcomes in our communities.

Main Responsibilities
  • Lead Clinical Practice: You will lead and monitor improvements to quality of care through clinical audit, evidence-based practice, clinical supervision, teaching, and supporting professional colleagues.
  • Implement Clinical Guidelines: You will lead the implementation of relevant clinical guidelines into practice and contribute to the further development of the evidence base through research and audit.
  • Develop Non-Medical Led Models of Care: You will lead the development of non-medical led models of care across Emergency departments, inpatient wards, intermediate care, care homes, and community teams in collaboration with multi-professional colleagues.
  • Improve Patient Flow: You will play a crucial role in improving patient flow through timely assessment, referral, responsive intervention, anticipatory care planning, and optimal discharge coordination across seven days.

Person Specification
  • Qualifications: Professional registration with NMC, HCPC, or GPhC, evidence of post-registration study, teaching qualification, PGDip in Advanced Clinical Practice (or equivalent), and Independent Prescribing MSc.
  • Knowledge: Research process and governance, audit process, management of patients with long-term conditions, management of patients with complex needs, local and national health policy, knowledge of public health issues, and ability to identify determinants of health in the area.
  • Skills and Abilities: Proficient in Microsoft Office and clinical data systems, excellent communication and interpersonal skills, ability to undertake risk assessment, autonomous practitioner, clinical examination skills, and advanced clinical practice skills.
  • Personal Attributes: Articulate, confident, decisive, motivated, ambitious, uses initiative, dedicated and flexible, self-aware, caring and compassionate, and demonstrates integrity and respect.
  • Ability to Manage: Ability to manage a caseload, able to cope with competitive demands in stressful situations, and manage change effectively.
