We are seeking a highly motivated and enthusiastic Health Check Advisor to join our team at Kent Community Health NHS Foundation Trust. As a Health Check Advisor, you will play a vital role in delivering high-quality health checks to eligible individuals across Kent, promoting health awareness and empowering individuals to make informed lifestyle choices.
Key Responsibilities- Deliver health checks to eligible individuals at clinics, community venues, workplaces, and outreach events across Kent.
- Take and record accurate physical measurements, including fingertip blood tests, to calculate an individual's risk of developing cardiovascular disease.
- Undertake meaningful conversations to communicate an individual's risk and encourage lifestyle changes using behavior change methodologies.
- Refer individuals for further investigation and/or lifestyle support as appropriate.
- Level 3 qualification in Health/Social care/Public Health or equivalent experience (2 years working in any of the above fields); RSPH Level 2 Health Checks; RSPH level 2 Understanding Health Improvement; RSPH level 2 Understanding Behaviour Change; Functional Skills at Level 2 (Literacy and numeracy/ GCSE grade A-C or equivalent in English and Maths)
- Previous experience of volunteering or working within a community setting; Evidence of being able to work autonomously; Experience of working in a setting with members of the public.
- Knowledge and experience of caring for, and empowering others respectfully; Working knowledge of Word, Excel, Outlook, and Internet.
