Position Overview

Service Care Solutions is seeking a dedicated Community Support Coordinator to enhance our team.

The ideal candidate will manage various forms of communication, including phone calls, written correspondence, and emails, ensuring that all customer inquiries, grievances, requests for information, and service demands are addressed effectively.

This role involves guiding clients through the understanding and completion of necessary documentation, forms, applications, and registrations via telephone, while also accommodating any specific needs they may have (such as disability, translation, or interpretation requirements).

Key Responsibilities

1. Evaluating referrals and implementing established eligibility standards.
2. Diligently documenting detailed referrals, outlining client requirements.
3. Adhering to established protocols, particularly concerning safeguarding vulnerable adults.
4. Offering information and guidance to the public from various backgrounds, focusing on a wide range of client demographics, including the elderly, individuals with physical disabilities, and those requiring occupational therapy.
5. Collaborating with both internal departments and external partner organizations as necessary.
6. Directing the public to additional services when suitable.

Qualifications and Skills

- Proven experience in a comparable position.
- Exceptional communication and interpersonal abilities.
- Capability to function effectively within a team environment.
- Proficient IT skills, particularly with Microsoft Office applications.
- Understanding of adult safeguarding principles.
- Competence to perform under pressure and meet deadlines.
