Support Coordinator

Job Summary
As a Support Coordinator at Home Group, you will play a vital role in supporting our customers who have complex needs to develop their skills and live more independently in the community. You will be responsible for signing new customers up to tenancies, supporting them to manage their tenancy, and carrying out risk assessments and support planning.
Key Responsibilities
- Develop and implement person-centred support plans to help customers achieve their goals and aspirations.
- Support customers to manage their tenancy, including preparing them for independent living.
- Carry out risk assessments and support planning to ensure customers' safety and well-being.
- Visit customers in their homes to provide support and guidance through the Home Achievement Program.
- Collaborate with colleagues to ensure seamless support and services for customers.
Requirements
- Passion for supporting customers to live their best life, with a focus on collaboration and attention to detail.
- Experience of creating and implementing person-centred support plans, as well as supporting colleagues in working to these plans.
- Experience of coordinating and assessing customer referrals, with a focus on ensuring the best possible outcomes.
- Ability to work independently, remain calm under pressure, and have a resilient approach.
- Valid driver's license and access to a vehicle insured for business purposes.
