Position Summary:
We are seeking a skilled Rehabilitation Support Coordinator to join our dedicated team in providing exceptional support to individuals with ongoing needs. This role involves working closely with both adults and children to enhance their quality of life and help them reach their full potential.

Key Responsibilities:
1. Advocate for clients' needs while fostering strong relationships with families, legal representatives, and other stakeholders.
2. Develop tailored action plans and objectives with clear timelines that align with the client's requirements.
3. Recruit, select, and oversee a team of care/support workers dedicated to client care.
4. Implement personalized Support & Rehabilitation Plans with specific guidelines for the support team.
5. Regularly monitor and evaluate the Case Management Plan and Support & Rehabilitation Plan, making adjustments as necessary to meet client needs.
6. Provide flexible working arrangements for the support team to ensure seamless service delivery, including availability during evenings, weekends, and public holidays.
7. Conduct risk assessments for key activities and establish risk management strategies to ensure the safety of all involved parties.

Qualifications:
- Hold a UK professional health qualification, such as HCPC, NMC, or Social Worker Registration.
- Possess a valid driving license and access to a vehicle.
- Previous experience in case management, particularly with adults or children, is preferred; however, comprehensive training will be provided.
- In-depth knowledge of CQC fundamental standards and key lines of inquiry.
- Familiarity with the Mental Capacity Act (MCA), Deprivation of Liberty Safeguards (DoLS), and safeguarding frameworks.
- Ability to address employee concerns in a fair and consistent manner.
- Strong organizational, time management, and planning skills.
- A flexible, professional, and positive approach to working across multiple sites.
