We are seeking a dedicated and proactive individual with a keen interest in patient care and rehabilitation to join our team as a Patient Journey Coordinator. The ideal candidate should possess a strong motivation, adaptability to change, and the ability to perform effectively under pressure. You will play a crucial role in ensuring that the patient experience throughout the healthcare system is seamless, minimizing any avoidable delays. Collaboration with key members of the multidisciplinary team (MDT) is essential to promote and facilitate safe and efficient discharges, thereby enhancing patient satisfaction and strengthening MDT relationships.

Key Responsibilities

The successful applicant will report to the ward manager, ensuring that the patient experience within the healthcare facility is as efficient as possible, without unnecessary interruptions. As this role is highly visible, it requires the individual to maintain a professional and courteous demeanor when interacting with colleagues and the public.

This position serves as a vital link among various members of the MDT, providing real-time updates on bed availability and discharge status. The role directly influences the enhancement of the patient journey, leading to increased patient satisfaction and a reduction in hospital stay duration.

Responsibilities Overview

1. Planning and Organizational Skills:

- Effectively prioritize your workload while managing your caseload and collaborating with other MDT members.

- Coordinate with the Senior Nurse on the ward to share pertinent information that supports safe and effective discharges.

2. Discharge Coordination

- Collaborate with the ward team to ensure clear communication across departments, driving timely and safe discharges by proactively completing all necessary administrative tasks.

- Participate in MDT meetings, documenting actions and discharge readiness to keep all staff informed of patient progress.

- Work closely with medical teams and pharmacy staff to ensure prompt prescription and dispensing of discharge medications.

- Liaise with Social Services and ward staff regarding patient needs, obtaining necessary consents to facilitate timely referrals in accordance with relevant legislation.

- Engage with care agencies to facilitate the resumption of care packages for patients whose needs have been assessed as appropriate for discharge.

- Actively participate in daily board rounds and complete timely documentation of data, following up on actions as required, while utilizing electronic systems to keep discharge readiness updated.

Qualifications and Skills

Essential Qualifications:

  • GCSE or equivalent in Mathematics and English, NVQ Level 3 or A Level equivalence.
  • RSA II typing or equivalent.

Experience and Knowledge:

  • Proficient computer skills.
  • Strong interpersonal skills.

Desirable Experience:

  • Familiarity with medical/nursing terminology.
  • Experience in a healthcare environment.

Skills and Abilities:

  • Excellent communication skills.
