About the Role

We are seeking a highly skilled and motivated Diabetes Specialist Nurse to join our dynamic and friendly Diabetes team for a 6-month fixed-term contract. As a key member of our team, you will be responsible for delivering patient-centered care and support to individuals with diabetes, promoting health and wellbeing, and empowering patients to self-manage their condition.

Key Responsibilities
  • Provide expert clinical care and support to patients with complex diabetes, enabling them to achieve optimum glycaemia control through informed self-management.
  • Empower and support patients to self-manage their condition, guiding them towards a 'normal' lifestyle and reducing morbidity and premature mortality by detecting and preventing complications.
  • Provide education and support to patients and their caregivers, promoting healthy lifestyle choices and reducing the risk of diabetes-related complications.
  • Collaborate with healthcare professionals and stakeholders to provide seamless care and support to patients with diabetes.
  • Develop and maintain strong relationships with primary care teams, ensuring effective communication and coordination of care.

Requirements
  • NMC Registration
  • Specialist Diabetes Qualification
  • Experience of working with patients with complex diabetes
  • Experience of participating in research and audits and disseminating evidence-based practice
  • Experience of teaching patients/carers and healthcare professionals, formally and informally
  • Experience of multi-disciplinary team working
