About the Role

We are seeking an experienced MDT Nurse to join our team at Sutton Primary Care Networks. As an MDT Nurse, you will play a vital role in providing high-quality care to patients with complex needs.

Key Responsibilities
  • Work collaboratively with GPs and other healthcare professionals to identify and manage patient needs.
  • Develop and implement personalized care plans to improve patient outcomes.
  • Provide education and support to patients and their families to promote self-management and independence.
  • Contribute to the development and implementation of clinical policies and guidelines.
  • Participate in multidisciplinary team meetings to discuss patient care and share best practices.
Requirements
  • Current NMC registration as a Registered General Nurse (RGN).
  • Experience working in a community or primary care setting.
  • Understanding of multidisciplinary team nursing roles and protocols.
  • Ability to work autonomously and as part of a team.
  • Excellent communication and interpersonal skills.
