Job Overview

We are seeking a highly motivated and experienced Diabetes Specialist Nurse to join our dynamic and friendly Diabetes team for a 6-month fixed-term contract. As a District Nurse Team Lead, you will play a crucial role in delivering patient-centered care and support to individuals with diabetes.

Main Responsibilities
  • Provide advanced assessment and advice on diabetes management to patients with complex diabetes needs.
  • Empower and support patients to self-manage their condition, promoting informed decision-making and healthy lifestyle choices.
  • Collaborate with healthcare professionals and stakeholders to ensure seamless care coordination and effective communication.
  • Develop and implement education programs to support patients and their caregivers in managing diabetes.
  • Contribute to the development and implementation of quality improvement initiatives to enhance patient outcomes and service delivery.
