About the Role

Sutton Primary Care Networks is seeking a highly skilled and experienced MDT Nurse Band 6 to join our team. As a key member of our multidisciplinary team, you will play a vital role in providing co-ordination and navigation of care and support across health and care services.

Key Responsibilities
  • Work closely with GPs and practice teams to manage a caseload of patients, acting as a central point of contact to ensure appropriate support is made available to people and their carers.
  • Support people to develop and review personalised care and support plans to manage their needs and achieve better healthcare outcomes.
  • Provide co-ordination and navigation for people and their carers across health and care services, helping to ensure patients receive a joined-up service and the appropriate support from the right person at the right time.
  • Work collaboratively with GPs and other primary care professionals within the PCN to proactively identify and manage a caseload, which may include patients with long-term health conditions.
  • Support the co-ordination and delivery of multidisciplinary teams with the PCN.
Requirements
  • Current NMC Registration RGN MDT Community Nurse Experience
  • Membership of a professional body RCN
  • Evidence of appropriate continuing professional development and education to maintain up to date knowledge
  • Minimum of 3 years post graduate experience in a community or primary care setting
  • Experience of working in a multidisciplinary team
  • Understanding of MDT nursing role in primary care
  • Understanding of protocols and clinical guidelines
  • Good understanding of managing complex and changing conditions in patients
  • Ability to manage change
  • Understanding of developing MDT Services
  • Ability to support patients to change lifestyle
  • Good Understanding of clinical governance
