Job Overview

We are seeking an enthusiastic and motivated Band 7 Diabetes Specialist Nurse to join our dynamic and friendly Diabetes team for a 6-month fixed-term contract.

About the Role

This is an exciting opportunity to work in a well-established, community-based, intermediate diabetes service led by a Diabetes Consultant. The service consists of Diabetes Specialist Nurses, Diabetes Specialist Dietitians, and dedicated administrative support.

Key Responsibilities
  • Deliver patient-centered advice, support, and care for patients with diabetes.
  • Provide advanced assessment of patients' needs and advise on appropriate diabetes management.
  • Work in both clinic and domiciliary settings, primarily with individuals with type 2 diabetes.
  • Collaborate with healthcare professionals and stakeholders along the diabetes care pathway.
  • Provide a link between the GP and consultant-led specialist teams.

Requirements
  • NMC Registration.
  • Specialist Diabetes Qualification.
  • Experience of working with patients with complex diabetes.
  • Experience of participating in research and audits and disseminating evidence-based practice.
  • Experience of teaching patients/carers and healthcare professionals, formally and informally.
  • Experience of multi-disciplinary team working.
