As part of our commitment to enhancing work-life balance, NHS Scotland is transitioning to a reduced full-time working week of 37 hours, effective from April 2024, without any alteration in remuneration. This adjustment will also apply proportionately to part-time staff. Please note that while this change is forthcoming, certain service areas may require a temporary extension of working hours for a limited period until they can fully implement the new schedule.

The Community Respiratory Team is a vibrant and adaptable multidisciplinary team dedicated to supporting patients with moderate to severe Chronic Obstructive Pulmonary Disease (COPD) in their homes throughout Glasgow City. Our team includes Physiotherapists, Nurses, Occupational Therapists, Pharmacists, Dietitians, Rehabilitation Support Workers, and various leadership and administrative roles.

This service is grounded in evidence-based practices, with the multidisciplinary team focusing on three key intervention pathways:

  • Prevention of Hospital Admissions: Providing support during acute exacerbations.
  • Early Supported Discharge: Facilitating timely discharge from hospital.
  • Chronic Disease Management: Emphasizing rehabilitation and self-management strategies.

The successful candidate will collaborate within the team to deliver comprehensive, timely, and individualized care. This includes assessment and treatment aimed at effective exacerbation management, preventing avoidable hospital admissions, maximizing patient functionality, and promoting self-management. You will also play a crucial role in supporting the early discharge of COPD patients from hospital settings.

Collaboration with the Respiratory Early Supported Discharge (ESD) team and hospital-based respiratory assessment services is essential. A degree in Physiotherapy and current registration with the HCPC are mandatory for this position. The standard working hours for this role are Monday to Friday, from 8:30 AM to 4:30 PM. Membership in Disclosure Scotland's PVG scheme is required.
