The position entails a shift pattern of 32 hours from Monday to Friday, with the possibility of weekend duties in the future.

In alignment with NHS Scotland's initiative, the full-time working week will transition from 37.5 to 37 hours starting April 2024, without any alteration in remuneration. This adjustment will also apply on a pro-rata basis for part-time employees. The current job listing and any subsequent employment offer will reflect these new working hours. However, due to varying service area capabilities, you may be required to work an additional 30 minutes per week temporarily, for which you will be compensated until your service area adapts to the new working week.

Position Overview: The Community Respiratory Team is a proactive and adaptable multidisciplinary team (MDT) focused on supporting patients with moderate to severe Chronic Obstructive Pulmonary Disease (COPD) in their homes throughout Glasgow City. The team consists of Physiotherapists, Nurses, Occupational Therapists, Pharmacists, Dietitians, Rehabilitation Support Workers, Business Support, and Leadership roles. This evidence-based service operates through three distinct intervention pathways:

  • Prevention of Hospital Admissions: Support for acute exacerbations.
  • Early Supported Discharge: Facilitating timely patient discharges.
  • Chronic Disease Management: Focused on rehabilitation and self-management.

The successful candidate will operate independently, utilizing their initiative within the framework of their existing knowledge and skills. They will work autonomously within their scope of practice to make clinical decisions regarding patient prioritization and management. Responsibilities include triaging work and allocating tasks based on clinical needs, leading triage decisions, managing referral allocations, and coordinating Consultant-led MDTs and safety huddles.

As a senior and advanced specialist within the team, the post holder will regularly deputize for the clinical lead or team lead. Collaboration with the Respiratory Early Supported Discharge (ESD) team and hospital-based respiratory assessment services/nurse specialists is essential. A First Level Registration on the appropriate adult nursing part of the NMC register is mandatory. Candidates should also have achieved or be working towards a non-medical prescribing qualification. Advanced assessment and decision-making capabilities at the Master's level or demonstrable advanced specialist decision-making skills through experience or professional development are also required.
