The position entails a shift pattern of 32 hours across Monday to Friday, with the possibility of weekend duties in the future.

In alignment with NHS Scotland's initiative to reduce the full-time working week from 37.5 to 37 hours effective from April 2024, this change will also apply on a pro-rata basis for part-time staff. Consequently, this advertisement and any ensuing employment offer will reflect the new working hours. However, due to varying service area capabilities, you may be required to work an additional 30 minutes per week temporarily, for which compensation will be provided until the service adapts its rosters or working patterns.

Role Overview: The Community Respiratory Team is a vibrant and adaptive multidisciplinary team (MDT) focused on supporting patients with moderate to severe Chronic Obstructive Pulmonary Disease (COPD) within their homes in Glasgow City. This team comprises Physiotherapists, Nurses, Occupational Therapists, Pharmacists, Dietitians, Rehabilitation Support Workers, Business Support, and Leadership roles. Our service is grounded in evidence-based practices, with the MDT facilitating three primary intervention pathways:

  • Prevention of Hospital Admissions: Support for acute exacerbations.
  • Early Supported Discharge: Assisting patients in transitioning from hospital to home.
  • Chronic Disease Management: Focusing on rehabilitation and self-management strategies.

The successful candidate will operate independently, utilizing their initiative within the scope of their existing knowledge and skills. They will autonomously make clinical judgments regarding patient prioritization and management. Responsibilities will include triaging work and allocating tasks based on clinical needs, leading triage decisions, referral allocations, and coordinating Consultant-led MDTs and safety huddles.

As a senior and advanced specialist team member, the individual will regularly deputize for the clinical lead or team lead. Collaboration with the Respiratory Early Supported Discharge (ESD) team and hospital-based respiratory assessment services/nurse specialists is essential. A First Level Registration on the appropriate adult nursing part of the NMC register is mandatory. Additionally, candidates should have achieved or be in the process of obtaining their non-medical prescribing qualification. Evidence of advanced assessment and decision-making skills at a Master's level or through professional development is also crucial.
