NHS Greater Glasgow and Clyde is a prominent healthcare provider in the UK, employing approximately 40,000 individuals across various clinical and non-clinical roles. We offer a comprehensive range of services, including acute hospital care, primary care, community health, and mental health services to a population exceeding 1.15 million, expanding to 2.2 million when considering our regional and national services.

The work schedule consists of 32 hours from Monday to Friday, with the possibility of weekend shifts in the future.

In alignment with NHS Scotland's initiative, the full-time working week will transition from 37.5 to 37 hours starting from April 2024, maintaining the same pay structure. This adjustment will also apply proportionately to part-time staff. The current job advertisement and any subsequent employment offers will reflect these new working hours. However, some service areas may not immediately implement the 37-hour workweek, and you may be required to work up to an additional 30 minutes weekly temporarily, for which you will be compensated until the service adjusts its schedules.

Role Overview: The Community Respiratory Team is a proactive and adaptable multidisciplinary team (MDT) dedicated to assisting patients with moderate to severe COPD in their homes throughout Glasgow City. The team comprises Physiotherapists, Nurses, Occupational Therapists, Pharmacists, Dietitians, Rehabilitation Support Workers, Business Support, and Leadership roles. Our service is grounded in evidence-based practices, supporting three primary intervention pathways:

  • Prevention of hospital admissions: Support for acute exacerbations.
  • Early supported discharge: Facilitating timely recovery.
  • Chronic disease management: Focused on rehabilitation and self-management.

The successful candidate will operate independently, exercising their initiative within the framework of their existing knowledge and skills. They will autonomously make clinical decisions regarding patient prioritization and management. Work will be triaged and allocated based on clinical needs, with the post holder frequently leading triage decisions, referral allocations, and coordinating Consultant-led MDTs and safety huddles.

As a senior and advanced specialist within the team, the post holder will regularly assume the responsibilities of the clinical lead or team lead. Collaboration with the Respiratory Early Supported Discharge (ESD) team and hospital-based respiratory assessment services/nurse specialists is essential. A First Level Registration on the appropriate adult nursing segment of the NMC register is mandatory. Candidates should have achieved or be pursuing their non-medical prescribing qualification. Evidence of advanced assessment and decision-making capabilities at the Master's level or demonstrated advanced specialist decision-making skills through experience or professional development is also required.
