Job Summary

NHS Scotland is seeking a highly skilled Physiotherapist to join our Community Respiratory Team. As a Physiotherapist - Chronic Disease Management Specialist, you will play a vital role in supporting patients with moderate to severe Chronic Obstructive Pulmonary Disease (COPD) in their own homes.

Key Responsibilities
  • Provide holistic, timely, and individualized care to patients with COPD, including assessment, treatment, and support for effective exacerbation management and avoidable hospital admission.
  • Work collaboratively with the Respiratory Early Supported Discharge (ESD) team and hospital-based respiratory assessment services/respiratory nurse specialists to ensure seamless care coordination.
  • Develop and implement individualized rehabilitation plans to maximize function and promote self-management in patients with COPD.
  • Conduct regular assessments and reviews to monitor patient progress and adjust treatment plans as needed.
  • Collaborate with other healthcare professionals, including nurses, occupational therapists, pharmacists, and dietitians, to provide comprehensive care to patients with COPD.
Requirements
  • Degree in Physiotherapy and current registration with the Health and Care Professions Council (HCPC).
  • Membership of Disclosure Scotland's Protecting Vulnerable Groups (PVG) scheme.
  • Excellent communication and interpersonal skills, with the ability to work effectively with patients, families, and healthcare professionals.
  • Strong problem-solving and critical thinking skills, with the ability to analyze complex patient data and develop effective treatment plans.
  • Ability to work independently and as part of a multidisciplinary team, with a strong commitment to delivering high-quality patient care.
