We provide comprehensive acute hospital, primary, community, and mental health services to a population exceeding 1.15 million, expanding to 2.2 million when considering our regional and national services.

As part of our commitment to employee well-being, NHS Scotland is transitioning to a reduced full-time working week of 37 hours, effective from April 2024, without any alteration in remuneration. This adjustment will also apply proportionately to part-time staff. Please note that while this change is forthcoming, certain service areas may require adherence to the previous 37.5-hour schedule temporarily.

The Community Respiratory Team is a proactive and adaptable multidisciplinary team dedicated to assisting patients with moderate to severe COPD within their homes in Glasgow City. Our team comprises Physiotherapists, Nurses, Occupational Therapists, Pharmacists, Dietitians, Rehabilitation Support Workers, and Business Support personnel.

This evidence-based service operates through three primary intervention pathways:

  • Prevention of hospital admissions (support for acute exacerbations)
  • Early supported discharge
  • Chronic disease management (rehabilitation and self-management)

The successful candidate will collaborate with the team to deliver holistic, timely, individualized, and clinically effective care. This includes assessment and treatment aimed at managing exacerbations effectively, preventing avoidable hospital admissions, maximizing patient functionality, and promoting self-management. You will also facilitate the early discharge of COPD patients from hospital settings.

Collaboration with the Respiratory Early Supported Discharge (ESD) team and hospital-based respiratory assessment services is essential. A Degree in Physiotherapy and current registration with the HCPC are mandatory for this position. The working hours for this role are Monday to Friday, from 8:30 AM to 4:30 PM. Membership in Disclosure Scotland's PVG is required.
