Leeds Intensive Support Team - Community Health Specialist

We are seeking a skilled Community Health Specialist to join our Leeds Intensive Support Team on a 12-month fixed-term contract. The successful candidate will provide vital support to individuals experiencing mental health or behavioral challenges, helping them to overcome their difficulties and maintain their independence.

The Intensive Support Team works collaboratively with individuals, their families, and other professionals to provide intensive support and empower individuals to achieve their goals. As a Community Health Specialist, you will be part of a multidisciplinary team, working closely with professionals to assess and implement care plans that meet the unique needs of each individual.

Main Responsibilities:

  • Collaborate with the multidisciplinary team to develop and implement care plans
  • Assess and address the behavioral and mental health needs of service users
  • Develop and deliver nursing care that promotes health and wellbeing
  • Participate in meetings and reviews related to service user care
  • Provide high-quality care and support to individuals with complex health needs

Key Requirements:

To be successful in this role, you will need:

  • Qualified Nurse (RNLD or RNMH) with experience working with individuals with mental health or learning disabilities
  • Excellent communication and collaboration skills

  • Knowledge of mental health and learning disability services
