About the Role

We are seeking a highly skilled and dedicated Community Nurse to join our Intensive Support Team at the Leeds and York Partnership NHS Foundation Trust. As a Community Nurse, you will play a vital role in providing high-quality care to individuals with learning disabilities and mental health needs.

Key Responsibilities
  • Work collaboratively with multidisciplinary teams to assess, plan, and implement care plans for individuals with complex needs.
  • Provide direct clinical care to adults with learning disabilities, with a focus on improving service users' lives and preventing hospital admissions.
  • Provide clinical supervision to junior colleagues and develop intervention toolkits for nurses.
  • Hold a caseload, including care coordination, to ensure care teams or family carers are supported to implement care and treatment as recommended.
  • Contribute to service development, service projects, and task groups, and participate in robust supervision structures.
Requirements
  • Must be a qualified nurse (Registration as RNLD or RNMH).
  • Experience of working with people with a learning disability.
  • Experience of working with a multi-disciplinary or multi-agency team.
  • Must be a driver with access to a vehicle.
  • Evidence of professional development.
