Community Nurse in Intensive Support Team

We are seeking a highly skilled and experienced Community Nurse to join our Intensive Support Team. As a key member of our multidisciplinary team, you will work closely with individuals with learning disabilities and complex needs, providing intensive support to help them achieve their goals and live fulfilling lives.
Key Responsibilities- Assess, plan, and implement care to meet the behavioural and mental needs of individuals with learning disabilities.
- Work collaboratively with professionals from various disciplines to ensure comprehensive care and support.
- Develop and maintain relationships with service users, their families, and carers to promote engagement and empowerment.
- Contribute to the development of care plans and interventions, ensuring they are tailored to individual needs and preferences.
- Provide clinical supervision to junior colleagues and support their professional development.
- Participate in service improvement initiatives and contribute to the development of new projects and initiatives.
