About the Role

We are seeking a highly skilled and compassionate Community Health Specialist to join our East North East Community Learning Disability Team. As a key member of our team, you will provide direct clinical care to adults with a learning disability, working to improve their lives and prevent admissions to specialist mental health and general health hospitals.

Key Responsibilities
  • Provide clinical and operational supervision to junior colleagues, developing intervention toolkits and holding a caseload of care coordination.
  • Develop and implement care plans, working closely with care teams and family carers to ensure the best possible outcomes for service users.
  • Contribute to service development, leading on projects and task groups to improve the quality of care and services.
  • Work collaboratively with multi-disciplinary and multi-agency teams to promote and advocate the health needs of adults with learning disabilities.
  • Participate in robust supervision structures, including 1:1s with the Specialist Nurse Practitioner and peer support through weekly MDT meetings and community nurse meetings.
Requirements
  • Qualified and registered RNLD/RNMH.
  • Experience of working with people with a learning disability and a multi-disciplinary or multi-agency team.
  • Must be a driver with access to a vehicle.
  • Evidence of professional development.
