About the Role

The Specialist Public Health Nurse will be responsible for promoting and improving health outcomes by identifying and prioritizing intervention based on complexity of need. This will involve working with children, young people, and families to develop and implement programs of care that are grounded in evidence-based theoretical and practical knowledge.

Key Responsibilities

  • Assess, plan, deliver, and evaluate patient-centered programs of care to a specialist level.
  • Work with children, young people, and families to identify and prioritize intervention based on complexity of need.
  • Develop and implement programs of care that are grounded in evidence-based theoretical and practical knowledge.
  • Collaborate with Children's Centres, Healthy Schools, and other key stakeholders to promote and improve health outcomes.
  • Engage with parents and young people in service design and development.
  • Work within evidence-based pathways and interventions, including young carers, young parents, infant mental health, economic wellbeing, drugs and alcohol misuse.

Requirements

  • Relevant qualifications and experience in public health nursing.
  • Excellent communication and interpersonal skills.
  • Ability to work autonomously and as part of a team.
  • Knowledge of evidence-based practices and interventions in public health nursing.
  • Ability to prioritize and manage a caseload.
