Leeds and York Partnership NHS Foundation Trust - Community Health Specialist

We are seeking a highly skilled Community Health Specialist to join our Intensive Support Team on a 12-month fixed-term contract. The successful candidate will provide vital support to individuals experiencing mental health or behavioral challenges, working collaboratively with a multidisciplinary team to empower individuals to overcome their challenges.

Main Responsibilities:

  • Collaborate with professionals to assess and implement care plans to meet behavioral and mental health needs
  • Develop and deliver nursing care to promote health and wellbeing
  • Participate in multidisciplinary team meetings and reviews to ensure effective care coordination
  • Provide high-quality care and support to individuals with specialist health needs
  • Contribute to service development and lead on projects within the team

About the Role:

The successful candidate will have a qualified nursing background, with experience working with individuals with learning disabilities and in a multi-disciplinary team setting. They will possess excellent communication and collaboration skills, with the ability to work effectively in a fast-paced environment.

  • Qualified Nurse (RNLD or RNMH)
