About the Role

We are seeking a highly skilled and experienced Community Nurse to join our Intensive Support Team at Leeds and York Partnership NHS Foundation Trust. As a Band 6 Community Nurse, you will play a vital role in providing direct clinical care to adults with learning disabilities, with a focus on improving service users' lives and preventing admissions to specialist mental health and general health hospitals.

Key Responsibilities
  • Provide direct clinical care to adults with learning disabilities, including assessment, planning, and implementation of care to meet their behavioural and mental needs.
  • Work collaboratively with a multidisciplinary team to develop and maintain the delivery of nursing care to promote health and wellbeing.
  • Lead in developing and maintaining the delivery of nursing care to promote health and wellbeing.
  • Maintain clear links with primary care and advise on mental health issues and support as necessary.
  • Participate in managing and prioritising time effectively.
  • Seek managerial supervision with Clinical Team Manager (CTM) effectively and efficiently.
Requirements
  • Must be a qualified nurse (Registration as RNLD or RNMH).
  • Experience of working with people with a learning disability.
  • Experience of working with a multi-disciplinary or multi-agency team.
  • Must be a driver with access to a vehicle.
  • Evidence of professional development.
