About Our Teams

ICMP East and West Teams

The ICMP provides a versatile service to all residents within the psychosis clusters aged 18 to 65 years, who currently require secondary community mental health treatment and support.

The service emphasizes social inclusion, recovery, self-management, and offers individuals choices regarding their treatment, while actively encouraging their involvement in the planning and execution of their care. This approach results in a personalized wellbeing plan that supports the individual's transition back to primary care.

Older Adults Community Mental Health Team

The Older Adults Mental Health team is a specialized integrated multidisciplinary team catering to adults over 65 experiencing severe and enduring mental health challenges or dementia. The team conducts health and social care assessments, provides support and advice for caregivers, and offers various interventions to enhance independent living and promote mental wellbeing.

The team delivers support through care coordination and a series of short-term psychological groups aimed at older individuals assessed for the service. All professionals collaborate with the inpatient unit, crisis, and assessment team to evaluate the support needed in the community, contribute to the multi-agency panel for those requiring care packages, and discuss safe hospital discharges.

Expectations for Candidates:

All social workers are expected to manage a caseload effectively and work collaboratively with service users, their families, and caregivers to ensure a strength-based, person-centered approach remains central to all interventions.

As part of a multidisciplinary team that includes social workers, occupational therapists, psychiatrists, psychologists, support workers, practitioners with lived experience, community psychiatric nurses, and administrative support, you will work autonomously with individuals, caregivers, and families to deliver a range of assessments and care interventions that promote mental health and personalized outcomes while fostering independence. You will be responsible for undertaking statutory functions of the social work role, including:

  • Social care assessments
  • Carers assessments
  • Court of protection
  • Safeguarding concerns and inquiries
  • Mental capacity assessments
  • Mental health assessments
  • Mental Health Act assessments

Senior social workers are expected to fulfill the role of an Approved Mental Health Professional (AMHP) and must act as an AMHP on behalf of the Royal Borough of Greenwich.

Essential Qualifications:

  • A registered professional social work qualification
  • An AMHP qualification
  • Registration with Social Work England
  • This role will require an Enhanced DBS check
