Same Day Access Coordinator

We are seeking a highly motivated and innovative individual to join our team as a Same Day Access Coordinator at Merchant Street Practice. This role is pivotal in ensuring all patients receive the best possible care and service.
Main Responsibilities- Coordinate all key activities, including access to services, advice, and information, to ensure timely, efficient, and patient-centered health and care planning.
- Support patient triaging and booking patients into practice slots, carrying out tasks from the GP, and monitoring the service.
- Work closely with practices within PCN 5 and 6 and the same day access team to provide a central coordinated role.
This is an exciting opportunity to join a collaborative team dedicated to delivering high-quality healthcare while initiating engaging initiatives to tackle the wider determinants of ill health.
Key Requirements- Experience in a reception/administration role, with evidence of consistent learning from education, training, and experience.
- Experience in Primary Care or social care, with the ability to communicate confidently with clinical and non-clinical staff.
- Good time management and prioritization skills, with the ability to work on own initiative and adapt to a pressurized environment.
- Ability to work with high volumes of patient contacts and maintain accurate patient records.
We are looking for a candidate who is committed to team working, has good IT skills, and is willing to undertake further learning and training.
