Clinical Practitioner

About the Role:
The Clinical Practitioner will be a key member of the multidisciplinary team at Kemple View, a 90-bedded hospital providing treatment and care for male patients with mental illness and/or personality disorder. The successful candidate will be responsible for providing high-quality patient care, working collaboratively with nursing and social work teams to promote excellent standards of care.
Main Responsibilities:
- Nursing:
- Act as the Nurse in Charge of the Ward, supervising a defined group of Nursing or HCA colleagues, and ensuring they receive management and clinical supervision.
- Prioritize nursing duties, delegate appropriately, and effectively work as nurse in charge as required, ensuring a safe environment.
- Recognize and respond appropriately to challenging behavior, supporting junior staff in managing difficult situations and developing resilience and strategies to manage.
- Monitor, evaluate, and report on patient care outcomes, working with complex patients and providing support to MDTs.
- Adhere to Company Medicines Management policies and Priory Group policies and protocols.
- Social Work:
- Provide accurate information about care in an accessible format to patients/service users and their families.
- Ensure an accurate, holistic, and up-to-date social history is available for each patient, offering formulations and recommendations.
- Initiate visits, maintain contact with patient's families and/or significant others, offering support, information, and joint family and patient sessions where relevant.
- Carry out focused casework with individual patients and their families using a range of interventions under supervision.
- Lead and coordinate the process of section 117 Mental Health Act aftercare plans with the MDT and relevant community agencies.
