We are seeking to recruit a highly motivated enthusiastic and forward thinking practitioner. The post holder will have have opportunity to build upon their existing clinical experience in a supportive environment offering ongoing clinical and professional development..

The post holder will work alongside individuals, their carers, support staff, Social Care, GMP, GMSST and other key stakeholders to ensure best person centred outcomes are achieved. The post holder will work autonomously and collaboratively within the skilled mixed integrated tea, completing highly specialist assessments whilst developing, implementing and reviewing specialist plans.

The Learning Disability and Mental Health Liaison Practitioner will provide a comprehensive service to people with a learning disability experiencing mental illness and having needs which require a collaborative MH/LD service response. The practitioner will work across both Pennine Care (PCFT) mental health and learning disability service areas to improve the outcome and experience of people accessing PCFT MH and/or LD services.
- Work collaboratively across a range of disciplines to continuously promote a ‘needs based’ approach to all service
- Promote an integrated approach to service
- Develop meaningful engagement with individuals who are often referred to as ‘difficult to engage’ and their
- Support local systems across the PCFT footprint proactively plan for people identified as at risk of admission (Transforming Care)
- Support through training, consultation and advice improvements in the overall capacity of MH services to support people with learning disabilities and realise benefits from accessing those services wherever appropriate.
- Promote both emotional and physical wellbeing for a designated caseload and as a area of improvement across all relevant service areas

- To carry a caseload and work with evidence based interventions to include: -- Assessment of need of the service user and carer
- Formulation of care plan
- Implementation of a plan of care for the service user in collaboration with the multi-disciplinary team, referring agent, family/carer and service user, where appropriate.
- Assessment and monitoring of risk
- To monitor, review and evaluate treatment and care in accordance with appropriate clinical frameworks
- To make timely referrals to appropriate services
- To support the commissioning of an appropriate package of care in the community by liaising with relevant
- To liaise with all agencies, and members of the multi-disciplinary team to ensure the needs of the service user are met and a culture of collaborative working is promoted.
- Establish and maintain communication with individuals and groups on the caseload about difficult or complex matters, in such a way as to overcome any problems in communication.
- To carry out clinic duties such as the Clozaril/ administration of depot injections either in the clients home or in the clinics. To manage medication and its administration within Trust Policy
- To supervise Community Support Workers / Band 5s as appropriate to implement elements of the care
- To maintain accurate, up to date records of all service users referred and receiving assessment, care treatment and support.
- To offer an initial holistic assessment to those newly referred to the service, and signpost as appropriate.
