Position Overview

NHS Professionals Limited is currently in search of a Community Rehabilitation Specialist to join our dedicated Rapid Response and Intermediate Care team. This role is essential in delivering high-quality rehabilitation services within the community setting.

Key Responsibilities

In this role, you will be responsible for:

  • Supporting the implementation of rehabilitation services across various units.
  • Providing both general and specialized care to adult patients, demonstrating proficiency across multiple professional disciplines.
  • Facilitating referrals to Allied Health Professionals or nursing services as necessary.
  • Acting as a vital member of the Rapid Response & Intermediate Care (RRIC) team, ensuring timely assessment and delivery of therapeutic interventions.
  • Managing specific components of patient treatment protocols and executing these treatments in patients' residences.
  • Serving as a technical resource for the Adult Community Healthcare Teams regarding mobility equipment provision.
  • Documenting and evaluating patient progress, collaborating with qualified Physiotherapists and Occupational Therapists post-intervention.
  • Overseeing the completion and potential delegation of non-clinical responsibilities within the RRIC and ACHT teams.

Essential Qualifications

To be considered for this position, candidates should possess:

  • Strong literacy and numeracy skills.
  • The ability to undertake training to develop necessary skills for the role.
  • Previous experience in healthcare assistance, rehabilitation assistance, or as an occupational therapy or physiotherapy assistant.
  • Experience in Community Intermediate Care.
  • A valid driving license and access to a vehicle for travel purposes.

Educational Requirements

Applicants should hold a relevant degree or be actively pursuing one.
