About the Role

We are seeking a highly skilled and motivated Community Health Specialist to join our team at Buckinghamshire Healthcare NHS Trust. As a key member of our Rapid Response and Intermediate Care service, you will play a vital role in providing expert clinical interventions and rehabilitation to patients in their own homes.

Your Mission

  • To provide high-quality, patient-centered care that promotes independence and prevents unnecessary hospitalization.
  • To work collaboratively with a multidisciplinary team to deliver effective care and support to patients and their families.
  • To contribute to the development and implementation of clinical pathways and protocols that improve patient outcomes.

About You

We are looking for an individual who is passionate about delivering high-quality care and has a strong commitment to excellence. You will have a proven track record of working in a community or in-patient setting and have experience of working with vulnerable adults and those who lack capacity.

Key Responsibilities

  • Providing comprehensive specialist assessments, reviews, and evaluations of patient needs.
  • Developing and implementing individualized care plans that promote patient independence and well-being.
  • Working collaboratively with a multidisciplinary team to deliver effective care and support to patients and their families.
  • Contributing to the development and implementation of clinical pathways and protocols that improve patient outcomes.
