Our forward-thinking Rapid Response & Intermediate Care service is growing, and we are looking for dedicated professionals to join our Urgent Community Response teams.

Your Objective: To deliver expert clinical assessments and rehabilitation as part of a cohesive team that responds promptly to individuals in urgent need. Your Goal: To assist patients in remaining in their homes, fostering recovery, and avoiding unnecessary hospital admissions. You will also facilitate early discharges from healthcare facilities and provide support for patients nearing the end of life. Your Responsibilities: Contribute your expertise to our multidisciplinary community team, collaborating with Physiotherapists, Occupational Therapists, Nurses, Specialist Paramedics, Paramedics, Pharmacists, and Community Healthcare Assistants. Main Responsibilities

Your primary responsibilities include:

Conducting triage, comprehensive assessments, and evaluations of patients and their caregivers. Managing urgent community responses to individuals in their homes to prevent hospital admissions and ensure patient safety through appropriate clinical interventions. Distributing workloads effectively among team members. Collaborating with various professionals to ensure timely and suitable care delivery. Exhibiting professional skills in leadership, care planning, coordination, and treatment delivery. Contributing to the development of clinical pathways.

Essential:

Experience

Essential:

Experience in relevant clinical areas, including care of the elderly, orthopedics, neurology, respiratory care, palliative care, and long-term conditions. Experience working within multidisciplinary teams.

Desirable:

Minimum of two years post-registration experience in community or inpatient settings. Familiarity with electronic clinical systems. Participation in research or audit projects. Experience in daily staff management and case allocation. Experience with single assessment processes.

Special Circumstances

Essential:

Possession of a valid driving license and the ability to travel to various locations as required.
