Position Overview

We are pleased to present an opportunity for a dedicated and experienced Clinical Nurse Consultant (CNC) to become a vital member of our community palliative care team. Our mission is to deliver exceptional, individualized care to individuals in need within our service area.

Work Schedule: Flexible part-time hours across 7 days (including weekends on a rotational basis)

Key Responsibilities

What does the role entail?

In this essential position, you will be tasked with effectively managing and coordinating a caseload of patients and their families who require specialized palliative and end-of-life care. You will conduct comprehensive assessments, collaborating with general practitioners, community nursing teams, and other healthcare professionals to ensure a flexible, responsive, and needs-oriented approach. Your focus will be on enhancing the quality of life for our patients, placing them at the heart of care during a critical phase of their journey.

You will deliver specialized palliative care to patients in their residences, as well as through virtual consultations and telephone support. Additionally, you will promote and enhance the specialist advisory service, utilizing your exceptional communication skills to influence the delivery of high-quality care.

Qualifications and Experience

Do you possess the following?

  • Registered Nurse with NMC
  • Relevant post-registration experience
  • Knowledge and experience in palliative care
  • Understanding of multi-disciplinary teamwork
  • Experience in reflective practice to enhance service delivery
  • Valid UK driving license and access to a vehicle for work purposes

It would be advantageous if you have:

  • Advanced clinical decision-making capabilities
  • Experience in both informal and formal teaching
  • Resilience and effective coping strategies for the emotional challenges of the role
