About the Role

We are seeking an experienced Community Nurse Specialist to join our team at Great Western Hospitals NHS Foundation Trust. As a Community Nurse Specialist, you will be responsible for providing high-quality care to patients in their own homes and community settings.

Key Responsibilities
  • Provide effective caseload management and holistic care to patients over the age of 18 requiring nursing care in their own homes and other community settings.
  • Work autonomously and independently, making decisions and judgments within a community setting.
  • Collaborate with the multidisciplinary team, including Community Matrons, Virtual ward, Tissue Viability Nursing, Continence Specialist, and other healthcare professionals.
  • Provide expert clinical advice, treatment, and support to patients with complex care needs or long-term conditions.
  • Manage and maintain accurate electronic patient records using SystmOne.
Requirements
  • RN (Adult) qualification.
  • Evidence of continuing professional development relevant for a Band 6 post.
  • Systematic approach to assessment, planning, and evaluation of needs-led care.
  • Ability to carry out activities of a fully functional community nurse, including end-of-life care, catheter management, and wound care management.
  • Previous community nurse experience and lone working advantageous.
