About the Role

We are seeking a highly motivated and experienced nurse to join our established heart failure service as a Heart Function Specialist Nurse - In Reach Training Post.

Key Responsibilities
  • Provide high-quality, individualized, evidence-based care to patients with heart failure and their families.
  • Act as a role model for others, supporting healthcare professionals in secondary and primary care to manage the complex needs of this patient group.
  • Develop and implement evidence-based guidelines for clinical practice.
  • Work collaboratively with the palliative care team to raise awareness of the need for palliation of symptoms.
  • Provide specialist advice and support to nurses and members of the multidisciplinary team.
  • Arrange follow-up care for patients admitted with heart failure.
  • Provide follow-up clinic as a specialist nurse in collaboration with Consultant Cardiologist.
Person Specification
  • First-level registration as a nurse.
  • Teaching and assessing qualification, ENB or City & Guilds 730 or equivalent.
  • Recognized cardiac care course.
  • Non-medical prescriber qualification or willingness to study for this.
  • Significant experience of working in a cardiac environment.
  • Experience of managing palliative/end-of-life care.
  • Ability to work as an effective member of a multidisciplinary team.
  • Demonstrated evidence of ability to educate others.
