Prospect Hospice is dedicated to providing exceptional palliative care to individuals in their homes, ensuring comfort and dignity throughout their journey.

We are seeking a compassionate and skilled Registered General Nurse to join our team of dedicated professionals. As a Palliative Care Nurse, you will play a vital role in delivering high-quality care to patients and their families, supporting them through challenging times with empathy and expertise.

Responsibilities
  • Maintain a comprehensive overview of the caseload, encompassing referrals, admissions, dependency status, and discharges.
  • Implement and review evidence-based practice guidelines to ensure best practices in palliative care.
  • Establish and maintain both electronic and paper documentation systems that support integrated, coordinated care and a consistent approach.
  • Promote the hospice's mission and values within Prospect Hospice and the wider community.
  • Provide direct clinical palliative care to patients in their homes. This includes identifying, assessing, and planning care tailored to their evolving needs.
  • Be prepared to deliver care in patients' homes, which may occasionally require night cover.
  • Develop strong relationships with healthcare professionals, social care providers, and community members to ensure seamless patient support.

Essential Skills:

  • Excellent communication skills, both written and verbal.
  • A commitment to continuous professional development and staying abreast of the latest advancements in palliative care. This includes actively identifying learning needs and pursuing relevant training opportunities for revalidation.
  • Strong organizational and time management skills.

  • Act as a positive ambassador for the hospice, providing patients and families with unwavering support through a calm, sensitive yet confident demeanor.
  • Create a welcoming and reassuring first impression. Demonstrate empathy and present a calm, courteous, and compassionate manner.
  • Demonstrate a genuine commitment to Prospect Hospice's values and behaviors at all times.
  • Have a proven track record of working inclusively and value diversity in all aspects of your work.
  • Maintain the highest professional standards at all times, serving as a role model for colleagues and upholding the hospice's reputation.

Desirable Qualities:

  • A collaborative team player with experience delivering accurate work within deadlines to contribute to team success.

Essential Qualifications:

  • Registered General Nurse (RGN).
  • Valid driver's license and access to a reliable vehicle throughout your shift to facilitate rapid response and home visits.
  • Current registration with the Nursing and Midwifery Council (NMC).

Essential Experience:

  • Proven experience in palliative care, demonstrating a strong understanding of multi-professional collaboration.
  • Confident in your knowledge base and able to recognize when further referral is necessary.
