Occupational Therapist Associate Practitioner

We are seeking a highly motivated and compassionate Occupational Therapist Associate Practitioner to join our Community Enhancing Recovery Team. As a key member of the team, you will work closely with our Occupational Therapist and Peer Support Worker to provide high-quality care and support to individuals with longer-term mental health conditions.
Main DutiesAs an Occupational Therapist Associate Practitioner, you will be responsible for facilitating care plans developed by the Multi-Disciplinary Team (MDT). You will work collaboratively with the care coordinators in the Enhanced Pathway to provide additional support to individuals and help them achieve their goals and aspirations.
You will need to demonstrate excellent communication and interpersonal skills, as well as the ability to work effectively in a team environment. You will also need to be able to maintain confidentiality and handle sensitive information with discretion.
Key Responsibilities- Deliver evidence-based planned care that is safe and effective, and enables service users to reach an optimum level of functioning, independence, rehabilitation, and recovery.
- Provide care within well-established policies, procedures, protocols, guidelines, and clinical care plans, while acting within the required sphere of clinical competences for the role at all times.
- Exercise initiative and judgment in care planning, goal setting, delivery, implementation, and evaluation of care.
- Work collaboratively and in partnership with others to develop knowledge and promote holistic care.
- Contribute to enabling service users to reach their potential, including the utilization of public health approaches, such as providing advice and support or signposting to the appropriate service.
We are looking for an individual who is passionate about delivering high-quality care and support to individuals with mental health conditions. You will need to have:
- A Level 5 Apprenticeship/Foundation degree in Health and Social Care or equivalent relevant to the post.
- A satisfactory sickness record over the previous 2 years.
- Able to fulfill Occupational Health requirements for the post (with reasonable adjustments if necessary).
- Excellent customer care skills, with the ability to work effectively in a team environment.
- Excellent communication and interpersonal skills, with the ability to maintain confidentiality and handle sensitive information with discretion.
- Ability to work flexibly to meet the needs of the service, including working on a 24-hour shift pattern.
