We are seeking a highly motivated and compassionate Occupational Therapist Associate Practitioner to join our Community Enhancing Recovery Team. As a key member of the team, you will work closely with our Occupational Therapist and Peer Support Worker to provide high-quality care and support to individuals with longer-term mental health conditions.
Main DutiesAs an Occupational Therapist Associate Practitioner, you will be responsible for facilitating care plans developed by the Multi-Disciplinary Team (MDT). You will work collaboratively with the care coordinators in the Enhanced Pathway to provide additional support to individuals and help them achieve their goals and aspirations.
You will need to demonstrate excellent communication and interpersonal skills, as well as the ability to work effectively in a team environment. You will also need to be able to provide care and support that is safe, effective, and compassionate, and to work within established policies and procedures.
Person SpecificationWe are looking for an individual who is passionate about making a difference in the lives of others. You will need to have a Level 5 Apprenticeship or Foundation Degree in Health and Social Care, or equivalent relevant to the post. You will also need to have experience of working in a caring environment at a band 3 or equivalent level of competence.
You will need to demonstrate excellent customer care skills, as well as the ability to work flexibly and respond to changing situations. You will also need to be able to maintain confidentiality and work within the Trust's values and policies.
