We are seeking a highly motivated and compassionate Occupational Therapist Associate Practitioner to join our Community Enhancing Recovery Team. As a key member of the team, you will play a vital role in helping individuals with longer-term mental health conditions to rehabilitate and reintegrate into their communities.
Main ResponsibilitiesAs an Occupational Therapist Associate Practitioner, you will be responsible for:
- Facilitating care plans developed by the multidisciplinary team (MDT) and working closely with the Occupational Therapist and Peer Support Worker.
- Delivering evidence-based planned care that is safe, effective, and enables service users to reach an optimum level of functioning, independence, rehabilitation, and recovery.
- Providing care within established policies, procedures, protocols, guidelines, and clinical care plans, while acting within the required sphere of clinical competences for the role.
- Working collaboratively and in partnership with others to develop knowledge and promote holistic care.
- Contributing to enabling service users to reach their potential, including the utilization of public health approaches, such as providing advice and support or signposting to the appropriate service.
We are looking for an individual who:
- Has a Level 5 Apprenticeship/Foundation degree in Health and Social Care or equivalent relevant to the post.
- Has excellent customer care skills, knowledge of care procedures, practice, recovery model, care pathway, and person-centered care.
- Can demonstrate initiative, decision-making, and prioritization skills, as well as excellent communication, numeracy, literacy, IT, and keyboard skills.
- Is able to work flexibly on a 24-hour shift pattern to meet the needs of service users.
- Has a non-judgmental, empathetic manner and is able to demonstrate Trust Values.
We are a specialist NHS Foundation Trust that provides community, mental health, and learning disability services for the people of Barnsley, Calderdale, Kirklees, and Wakefield. We also provide low and medium secure services and are the lead for the West Yorkshire Secure Provider Collaborative.
