Position Overview

An exceptional opportunity has emerged for a dedicated and qualified District Nurse to step into the role of Team Leader for the Lightwood Central service.

The successful candidate will oversee, guide, and mentor a Community Nursing Team, promoting research and evidence-based practices. They will spearhead the assessment process and ensure the delivery of high-quality nursing care within the community, as part of a broader integrated care framework. Additional responsibilities include ensuring that service delivery aligns with the Core Offer and the Standard Operating Procedures of the Community Nursing Service.

This expansive and dynamic team is integrated within the wider Community Care structure, collaborating to provide exceptional, personalized patient care in home settings.

Main Responsibilities

  • Assess and prioritize the health requirements of the Community Nursing caseload, while being attuned to the health needs of the practice population and contributing to the local public health agenda.
  • Manage the daily operations of the District Nursing Team.
  • Lead, coordinate, and evaluate care programs tailored to the needs of individual patients, caregivers, and families.
  • Accountable for the performance of the District Nursing Team and its members.
  • Encourage self-care principles and empower patients.
  • Collaborate with other agencies to act as care manager for designated patients, aiming to prevent unnecessary hospital admissions.
  • Effectively manage resources and provide guidance on skill mix requirements for the team, in partnership with the line manager, while overseeing team budgets.
  • Actively engage in and promote Clinical Governance to maintain the highest quality of practice within the service, demonstrating evidence-based practice and participation in clinical audits.
  • Support the team in delivering care to patients in their homes.

Qualifications

Essential

  • Community Specialist Practitioner (District Nurse) degree.
  • Registered Nurse, first level with a valid MNC registration.
  • Nurse prescriber (V150).
  • Evidence of ongoing continuous professional development.
  • Mentorship/Preceptorship in Chronic Disease Management.
  • Teaching and assessing qualification or equivalent experience.
  • Clinical Practice Educator or willingness to work towards this qualification.

Desirable

  • Non-Medical Prescriber (V300).
  • Clinical Practice Educator Qualification.
