We are seeking a visionary nurse to join our Integrated Hospital Discharge team at The Princess Alexandra Hospital NHS Trust.

You will have a keen interest in complex discharge planning, proven communication skills, and be able to work collaboratively with patients, carers, relatives, and system partners.

As a team, we are committed to enhancing the patient experience of discharge planning services which ensure that patients' time and wishes are valued.

Main Responsibilities

The Integrated Hospital Discharge Team (IHDT) provides expert discharge planning advice and support for patients across the trust who require support from community services on discharge.

The main duties of the role will be to plan, provide, and maintain effective Discharge Planning care to patients/clients and their relatives.

To continuously develop and enhance knowledge and skills that will directly influence consistent standards of Discharge Planning practice.

Qualifications

  • Registered Nurse (Part 1)
  • Post Graduate Degree
  • Evidence of continued Professional development

Experience

  • Evidence of addressing issues related to clinical governance
  • Utilise research to support clinical and discharge planning practice
  • Previous experience of discharge planning practice
